My current thoughts:

Sorry for Academy City showing up multiple times, but it's a central part of the setting and is both a location and an organization.

My reasoning for the characters was the three heroes, the unofficial fourth (Mikoto), and the main heroine in the series' title, then categories of characters. Aleister Crowley is the central antagonist that created the setting in a way so he is on his own. Almost added Junko to the heroes because she is now a protagonist with her own series, but if I added every people who got to be protagonist of side stories (Kuroko, Kanzaki, Styil, Itsuwa, etc.), it would never end.

I wasn't sure about how to go for organizations so I went for all encompassing categories: AC, the Churches, then the more individual magic groups. I wanted to add the orgs important to the heroes and series in more detail, but it would have bloated the result (Judgment, Necessarius, Skill-Out, GROUP/rest of Dark Side, Kihara, etc.), so I went in a more general direction.

Finally, I tried to restrain the items in the magic/science sections to terminology or concepts that would be central to the series' plot.

You can ask any further questions if my list is still confusing or if you want me to explain my reasoning.
